I'm in manual mode with a 40D and I set a sequence of 7.
Do I set the exposure to the middle or "good" exposure?
Do I expect to have 3 shots under the exposure I set and 3 over?
What sequence are the brackets taken in (least exposed to most, most to least, or some other?)

You should set the initial manual exposure to the best middle exposure. Auto-bracketing will start at the longest exposure and finish with the shortest.
e.g. a 7 shot sequence with 1 stop intervals with manual exposure set to 1/125 @ f/8 will take the following sequence:

1/15 @ f/8
1/30 @ f/8
1/60 @ f/8
1/125 @ f/8
1/250 @ f/8
1/500 @ f/8
1/1000 @ f/8
